‘No link between MMR and autism,’ major study finds

“The MMR jab does not lead to autism: Scientists debunk controversial theory yet again,” reports the Mail Online.

A major study has confirmed yet again that there’s no link between autism and the MMR vaccine, which protects against measles, mumps and rubella.

The vaccine is given as part of the routine childhood immunisation schedule in the UK. The first dose is given around the time of a child’s first birthday and the second at 3 years and 4 months.

Take-up of the vaccine was affected after the publication of a controversial study by Andrew Wakefield in 1998, which claimed there was a link between the MMR vaccine and autism.

But Wakefield’s work has since been completely discredited and he has been struck off as a doctor in the UK.

Subsequent studies in the last 9 years have found no link between the MMR vaccine and autism.

In this latest study, researchers followed up 650,000 Danish children until they were on average 8 years old. Researchers found around 1% of them developed autism.

Most children in the study had received the MMR vaccine. There was no difference in the rates of autism between those who’d been vaccinated and those who had not.

Nor was there any link with the MMR vaccine when looking at children who may be at higher risk of autism, such as those with a sibling with autism.

This study strongly supports the fact that the MMR vaccine does not cause autism.

Where did the story come from?

This study was conducted by researchers from Statens Serum Institut and the University of Copenhagen in Denmark, and Stanford University School of Medicine in the US.

It was funded by the Novo Nordisk Foundation and the Danish Ministry of Health.

The media coverage was fairly accurate.

What kind of research was this?

This was a population-wide cohort study in Denmark that aimed to investigate the now-discredited link between the MMR vaccine and autism.

Observational studies like these are commonly used to investigate a link between a potential exposure (in this case, the vaccine) and an outcome (autism) and have the advantage that they can follow very large numbers of children over many years.

What did the research involve?

The study used the Danish Civil Registration System to identify all children born in Denmark from 1999 to 2010.

In Denmark, the first dose of MMR is given at 15 months and the second dose at 4 years of age. Before 2008, the second dose was given at 12 years old.

The researchers noted whether each child had received both MMR disease and other childhood immunisations.

Information on autism was obtained through the Danish Psychiatric Central Register, which codes the individual diagnoses as given by psychiatrists, such as autism disorder, Asperger’s or other developmental disorder.

Using the Danish National Patient Register, they also reviewed any diagnoses for medical conditions.

In their analyses, they adjusted for other factors that could be linked with an autism risk, such as:

  • parents’ age
  • smoking during pregnancy
  • premature birth
  • low birth weight and head circumference
  • multiple birth
  • vital signs and physical health in the newborn

What were the basic results?

A total of 657,461 children born from 1999 to 2010 were followed up between 2000 and 2013 to an average of 8.6 years of age.

The vast majority had received the MMR vaccine, with only 5% (31,619 children) not being vaccinated.

A total of 6,517 children developed autism, which is a rate of around 1 in 100 children during 10 years of follow-up. Diagnosis was at around 6 or 7 years of age on average.

When comparing MMR-vaccinated and non-vaccinated children, there was no link between MMR and autism risk (hazard ratio 0.93, 95% confidence interval 0.85 to 1.02).

Results were similar when grouping children according to their age (how long since they had the MMR jab) and whether or not they’d received other childhood vaccinations.

Autism diagnosis was more common among boys and children who had siblings with autism.

When looking at sub-groups of boys, those with siblings with autism or other high-risk factors for autism, researchers still found no link with the MMR vaccine.

How did the researchers interpret the results?

The researchers concluded: “The study strongly supports that MMR vaccination does not increase the risk for autism, does not trigger autism in susceptible children, and is not associated with clustering of autism cases after vaccination.”

Conclusion

This study supports earlier studies that the MMR vaccine has no link with the risk of autism.

It follows a 2014 review that pooled the results of 10 observational studies on childhood vaccine and found no evidence of any link between the MMR vaccine and autism.

The strength of this study is that it follows a large number of children. This makes the findings more reliable when assessing a fairly rare outcome like autism, and reduces the possibility that the findings are down to chance.

The analysis of this study specifically looks at susceptible sub-groups and whether there could be any clustering of cases after the vaccine is given, something that the controversial 1998 Wakefield paper suggests.

But in all analyses, it’s clear the MMR vaccine does not increase the risk of autism.

The suggested autism links in the 1998 paper were completely unjustified and not backed by evidence. Yet despite the paper being withdrawn, it continues to cause harm.

There have been several measles outbreaks in Europe and the US, and avoidance of the MMR vaccine has been identified as a major cause.

Studies have found that concerns over an autism link are the main reason parents avoid giving their child the vaccine.

It’s hoped that this large study will reassure the public and health professionals that the MMR has no link at all with the risk of autism, either in the child population overall or in potentially higher risk children.

“There is no evidence whatsoever linking the development of autism to childhood vaccines,” The Guardian reports.

A new study involving more than a million children found no evidence of a link between childhood vaccines and autism or autism spectrum disorder.

Researchers pooled the results of studies that have assessed the relationship between vaccine administration and the subsequent development of autism spectrum disorder. No significant associations were found between vaccinations and the development of the condition.

The results of this study therefore suggest that there is no reason that parents should avoid having their child vaccinated because of fears that their child will develop autism after they are immunised.

As a result of the success of the NHS childhood vaccination programme, many parents think that childhood diseases such as mumps and measles are a thing of the past and not a threat to health. But this couldn’t be further from the truth.

Because of a decline in vaccine uptake, there was a measles outbreak in Wales in 2012 involving 800 confirmed cases of measles, including one death.

The potential complications of conditions such as mumps and measles are serious, and include meningitisencephalitis (brain infection), loss of vision, infertility, and even death.

 

Is autism becoming more common?

Many anti-vaccination campaigners point to the increasing number of children being diagnosed with autism spectrum disorder as “proof” that vaccines cause autism.

 

But it is far more likely that doctors have simply got better at diagnosing the condition.

Where did the story come from?

The study was carried out by researchers from the University of Sydney.

The source of funding was not reported. The authors reported that they had no conflicts of interest.

It was published in the peer-reviewed medical journal, Vaccine.

The results of this study were covered well by the UK media.

 

What kind of research was this?

This was a systematic review and meta-analysis of case-control and cohort studies that assessed the relationship between vaccine administration and the subsequent development of autism or autism spectrum disorder.

A systematic review is an overview of primary studies. Systematic reviews use explicit and reproducible methods to search for and assess studies for inclusion in the review.

A meta-analysis is a mathematical synthesis of the results of the studies included.

This is an appropriate way of pooling and studying the body of available evidence on a specific topic.

 

What did the research involve?

The researchers searched databases of published literature to identify case-control and cohort studies that have assessed the relationship between vaccine administration and the subsequent development of autism or autism spectrum disorder.

Studies were included that looked at measles, mumps and rubella (MMR) vaccination, cumulative mercury, or cumulative thimerosal dosage from vaccinations. Thimerosal is a mercury-containing chemical that acts as a preservative.

Once the studies were identified, the researchers assessed the quality of the study to see if there was any bias, and extracted data about the study characteristics (study design, the number of participants, the type, timing and dose of vaccine, and outcome) and its results.

The researchers then performed a meta-analysis to combine the results of the studies included in the review.

 

What were the basic results?

The researchers included five cohort studies involving 1,256,407 children, and five case-control studies involving 9,920 children.

None of the five cohort studies found an association between vaccination and autism or other autism spectrum disorder. When the results of the five cohort studies were combined, there was no increased risk of developing autism or autism spectrum disorder after MMR, mercury or thimerosal exposure (odds ratio [OR] 0.98, 95% confidence interval [CI] 0.92 to 1.04).

The researchers performed subgroup analyses looking at autism and autism spectrum disorder separately. There was no increased risk of developing autism (OR 0.99; 95% CI 0.92 to 1.06) or autism spectrum disorder (OR 0.91; 95% CI 0.68 to 1.20).

They then performed subgroup analyses looking at the different exposures separately. There was no increased risk of developing autism or autism spectrum disorder after MMR vaccination (OR 0.84; 95% CI 0.70 to 1.01), thimerosal exposure (OR 1.00; 95%CI 0.77 to 1.31), or mercury exposure (OR 1.00; 95% CI 0.93 to 1.07).

The researchers also looked at whether there was any evidence of publication bias, a bias that can occur if the publication of research results depends on the nature and direction of the results. Typically, positive results tend to get published while negative results get shelved. If this occurs, it can distort the results of systematic reviews and meta-analyses.

The researchers analysed the results of the cohort studies using statistical tools and found no evidence of publication bias.

Similarly, none of the five case-control studies found an association between vaccination and autism or autism spectrum disorder individually or when combined, and none of the subgroup analyses found any associations.

 

How did the researchers interpret the results?

The researchers concluded that, “The findings of this meta-analysis suggest that vaccinations are not associated with the development of autism or autism spectrum disorder.

“Furthermore, the components of the vaccines (thimerosal or mercury) or multiple vaccines (MMR) are not associated with the development of autism or autism spectrum disorder.”

 

Conclusion

This systematic review and meta-analysis has found no association between vaccination and the development of autism or autism spectrum disorder. The cohort studies included in the systematic review had information on more than a million children from four different countries.

This was a valuable and rigorous piece of research that will hopefully reassure parents who have any concerns about getting their children vaccinated against childhood diseases.

As with all studies, this research has limitations. It excluded data from the Vaccine Adverse Event Reporting System (VAERS) in the United States, a system similar to the Yellow Card scheme in the UK.

This was because researchers say that VAERS data has many limitations and a high risk of bias because of unverified reports, under-reporting, inconsistent data quality, absence of an unvaccinated control group, and many reports being filed in connection with litigation.

It is unclear what effect including these studies would have had on the results of the meta-analysis.

Overall, however, this study adds to the body of evidence that proves that the benefits of vaccination far outweigh any risk.

Others argued the opposite and said there was no link between vaccinations and autism.

Like many of my readers I have been following the recent Ebola outbreak with some concern. I was send the following information this morning which I thought would be useful to share.

Unless Ebola control measures in West Africa are enhanced quickly, experts from the World Health Organization (WHO) and Imperial College, London, predict numbers will continue to climb exponentially, and more than 20,000 people will have been infected by early November, according to a new article in the New England Journal of Medicine released six months after WHO was first notified of the outbreak in West Africa.

In the article, public health epidemiologists and statisticians reviewed data since the beginning of the outbreak in December 2013 to determine the scale of the epidemic, better understand the spread of the disease, and what it will take to reverse the trend of infections.

Scale of epidemic

Although WHO was first notified of the outbreak on 23 March 2014, investigations retroactively revealed the outbreak started in December 2013. Between 30 December 2013 and 14 September 2014, a total of 4507 cases were reported to WHO.


The data in the study help clarify some details of who is most affected by this outbreak. For example, there have been mixed reports on whether women might be harder hit because they are more likely to care for sick, or whether it would be men who might be more likely to bury the highly-infectious dead bodies.

“This study gave us some real insight into how this outbreak was working, for example, we learned there is no significant difference among the different countries in the total numbers of male and female case patients,” says Dr Christopher Dye, Director of Strategy for WHO, and co-author of the study. “There may be differences in some communities, but when we actually looked at all the data combined, we saw it was really almost split 50-50.”

The extensive review of data also allowed for a closer look at case fatality rate.

“Assessing the case fatality rate during this epidemic is complicated by incomplete information on the clinical outcomes of many cases, both detected and undetected,” says Dye. “This analysis shows that by 14 September, a total of 70.8% of patients with definitive outcomes have died. This rate was consistent among Guinea, Liberia and Sierra Leone.”

But the case fatality rate was lower when only hospitalized patients were considered, supporting evidence that getting patients to good, supportive health care quickly makes a difference.

Spread of infection

The examination of the data also showed the spread more clearly. In late December, the first cases were reported in the forest areas of Guinea. By March, when the government sounded the alarm to WHO, cases had already spread from the forest area to the capital of Conakry. In May, the focus of the outbreak in Guinea expanded strongly to Sierra Leone and in June it really took hold in Liberia. From July onward, there were sharp increases in case numbers in all three countries.

Projections

Although the current epidemic in West Africa is unprecedented in scale, the clinical course of infection and the transmissibility of the virus are similar to those in previous Ebola outbreaks.
“We infer that the present epidemic is exceptionally large, not primarily because of biologic characteristics of the virus, but in part because of the attributes of the affected populations, the condition of the health systems, and because control efforts have been insufficient to halt the spread of infection,” says Dye.

There are challenges in this region that exacerbate the struggles to contain the virus quickly. Most importantly the health systems in all three countries were shattered after years of conflict and there was a significant shortage of health workers, leaving the system weaker than in other countries with Ebola outbreaks. In addition, certain characteristics of the population may have led to the rapid spread of the disease, for example, the populations of Guinea, Liberia, and Sierra Leone are highly interconnected, with extensive cross-border traffic at the epicentre and relatively easy connections by road between rural towns and villages and the densely populated capital cities.
“The large intermixing population has facilitated the spread of infection, but a large outbreak was not inevitable,” says Professor Christl Donnelly, Professor of Statistical Epidemiology, Imperial College and the MRC Centre for Outbreak Analysis and Modelling. “In Nigeria, for example, where health systems are stronger, the number of cases has so far been limited, despite the introduction of infection into the large cities of Lagos and Port Harcourt.“

The critical determinant of outbreak size appears to be the speed of implementation of rigorous control measures.

“Forward projections suggest that unless control measures – including improvements in contract tracing, adequate case isolation, increased quality of care and capacity for clinical management, greater community engagement, and support from international partners – improve quickly, these three countries will soon be reporting thousands of cases and deaths each week,” says Dye.
Experimental therapeutics and vaccines offer promise for the future, but are unlikely to be available in the quantities needed to make a substantial difference in control efforts for many months, even if they are proved to be safe and effective.

The risk of continued expansion of the Ebola outbreak is real. This study provides the evidence needed for an urgent wakeup call requiring intensive scaling up of control measures while working towards rapid development and deployment of new medicines and vaccines.

The debate about vaccinations and autism has been raging for over a decade now.  It is a highly contentious issue with very strong feelings on both sides.

We would like to take a “snapshot”  of the current views of our readers on the subject.  To this end we have created a poll which we would ask you to take – which you can find at the end of this post.

SpeaksAspergian I was diagnosed with Asperger’s/autism when I was 58 years old.  I am a geneticist and a conservation biologist.  I would love to say more about myself, but I come to this conversation not even knowing what phrases are acceptable. I come to a community that I wish would accept my differences, because I honestly do not understand the politics of how we are supposed to talk about autism.

ALL that being said, I would like to write a wee bit about the genetics, so that we are clear on what is known, which is far from enough. I apologize in advance if my phraseology is politically incorrect, but my limitation is this: I try to be pc, but I am hardwired to communicate in my way.

There are clearly, and understandably, misunderstandings in genetics which I have noticed in the comments.  Here is some big picture stuff:  Autism falls into the category of “quantitative” genetics. This means that there are many, many genes involved, each with a small effect, each with a small contribution to the whole “phenotype” that we call autism.  (I use the neutral scientific term “phenotype” instead of disease or syndrome, even though I believe that these other terms may be used correctly in appropriate context.)  What this means is that autism is NOT like cystic fibrosis or sickle cell anemia, which are human diseases caused by a mutation in a single gene.  Autism is literally based on a spectrum of genes, many, many genes. I cannot stress enough that many genes are involved and that we need much more scientific research.
According to statistics, autism has a substantial genetic basis AND is influenced by environment.  Both are important.  Here’s a personal example:

I was conceived in a little house beside a golf course in the 1950’s (read: pesticides without modern controls).  Because I am the first person in my family “with autism” (sorry to those who dislike the “with” phrase), and because biological development is altered in autism, I have asked, could those chemical toxins that my mother may have breathed over 9 months TIPPED my genetic cart (the genes that were there already in me) in such a way that I developed Asperger’s?  Maybe and maybe not. This question cannot yet be answered.  We don’t know because it is possible that my Asperger’s (may I speak it this way?) would have developed anyway!  Many little genes of small effects can together make a big effect.

What about vaccines?  I have not read the actual clinical trials, but I accept the reported result that there is not a statistical link. This does not mean that I don’t think there is something else in our environment TIPPING THE CART for many people.  We are all carrying genes that matter in autism.

I say this because of the very sophisticated “genetic architecture” of autism.  Autism as a phenotype may be a modern syndrome, but its genetics have evolved in our human population over a very, very long period of time.  We need research as well as _______(the pc word for treatment … anybody?).

My hope is to help bring the science of autism genetics to the people.  This is certainly not an easy task, not only because to render that science accessible will take much work, but because I am not wired to be politically correct.  My hope is that the controversies, protests and boycotts will give way to a unified research front going forward.  My hope is that readers will accept my limitation in how I use language, and let me write about genetics  for the general public without criticizing my Aspie inability (am I wrong to use ‘inability’?) to use language phrases that are not my own.

MarkAllen2 Donna Loveless totally agree donna…….the spectrum is so broad now that it has become near impossible to pin point a direct cause…… what i don’t buy into is the government (new zealand) saying that the majority of autism is genetical…….this is a straight out lie, genetics at most only accounts for no more than 30%, the birthing process between 5& & 15% ……. that means that anywhere between 55% & 65% is environmental!!!  Vaccines can and has caused autism, this has been verified in both US and Italian Courts where the Judges have ruled just that – the fact that the government then seals the case so the public cannot get access to the evidence, to me, is their submission of the FACT!  However, like i said before, the spectrum is now so broad – and basically each case and child is different/unique – to me the environmental factors explains the rest………..toxics/poisons such as pesticides, herbicides, fluoridated water, lead, mercury, aluminium. round-up, lead, arsenic, etc etc the list goes on and on which to me, at least, accounts for the wide variety of symptoms that fall on the spectrum……….my personal experince with one of my boys (and only 1, the middle of 5) tells me that it was his vaccine that triggered/caused his autism……i have home video of him 3-4 months before his shot………..alert, verbal, responsive……..i even have a plunket nurse report stating that she had ZERO concerns about is ‘development’ just 1 month prior to his shot…………it was a very slow regression for my son, being with him everyday i wasnt the one to notice and raise the alarm bells…..rather it was my mother who visits every school holiday, every 3 months that noticed……the rest is history and the only benefit for my family is that this woke me the f%$k up and i now question EVERYTHING and if my question dont get answered i simply do not CONSENT to ANY medical/government/authority and NEVER WILL!  My family – My choice

I’ve done so much reading on the vaccine/autism debate. However in my opinion, I have proof right in front of me of no link. Autism is not a disease to be “cured”. It a developmental/personality disorder that occurs in utero. Yeah I think genetics plays a big part in it too. Lots of Autism diagnosis in my family. Maternal and paternal. My daughter knows the diagnosis doesn’t change a thing about her. She knows she’s perfect who she is. I just don’t see any legitimate proof to the link. But I do not ever claim to be an expert. This is all just MY opinion.

Anytime a person/child comes in contact with infected blood the risk is present. Yes, sex and sharing needles…….. no vaccines in my ancestors or myself, so why do most of us have autism? my children were vaccinated but not at birth and that is when the autism was obvious to their autistic mother (diagnosed at age2) My grandchildren got all the nasty crap like HepB and they are past age 3 with NO SIGNS of autism. anecdotes are not science but I will never believe the damage done by the vaccines is autism. there could a link because autistics tend to be more sensitive to and unable to detoxify as well as the allistic. I am for spacing out the vaccines for autistics and a couple I am against but it has nothing to do with autism. that is ridiculous!

amandapowell1 The toxins in vaccines are a large contributor to autism.  I don’t think they caused my daughter’s autism all by themselves, but I also don’t think she’d have it today without them.  Autism didn’t come until her 12 month shots.  You will not ever meet an autistic individual who never had vaccines and who’s parents and grandparents never had vaccines.  Unfortunately heavy metal and other toxins to pass on from both parents to the fetus so they harm the baby.  That is why some kids are “born with autism”.  They did not have it from conception.  the toxins damaged them throughout the 9 months they were in the womb.  This also explains why some kids aren’t vaccinated and still get autism.  The toxins damage their stomachs, leading to brain problems since the gut and brain are connected.

HeatherSaggers Both my sons have autism, one is ‘worse’ than the other. I say worse, but he is more obvious than my other son. My husband is autistic and i am pretty sure his dad is also autistic. For me that says it is in the genes somewhere. But one thing i will always say is, that i would rather give my child a vaccination than let them catch a preventable disease and potentially die from it.

The thing about online access is if you believe something then the online world will give you the answers your looking for. For me i recently was in pain, i looked on the internet and everything i pulled up told me i had cancer. It made me worry, the symptoms fit. Yet it ended up being a pulled muscle. If you want to find that vaccines cause autism that’s what you will find online. But you only see what you have decided. I firmly believe vaccines do not CAUSE autism
